2012 SPECIAL DISABILITY PROVISIONS FOR EXTERNAL EXAMINATIONS
The Tasmanian Qualifications Authority provides for special assessment arrangements to be made for students who, because of their diagnosed disability, are unable to be assessed in the same way as other students.
Please complete the Request for Special Examination Arrangements form detailing what special external examination arrangements or special assessment considerations will be required for students with disabilities from your school or college. One form per student should be completed.
This form is available from the TQA web site or can be downloaded directly from this message.
It should be noted that the forms of those students who require a re-written examination paper because of their specific circumstances (ie, legally blind or deaf) MUST be notified to the TQA BY NO LATER THAN THE END OF TERM 1.
All other forms should be returned to the Authority by NO LATER THAN THE END OF TERM 2.
Only emergency cases (such as a physical injury that occurs after the end of Term 2 that will impair a student undertaking their examinations) will be considered from that point for possible consideration for the 2012 examinations.

This form is only to be used for students with a diagnosed disability who requires special arrangements to be made for their examinations. If a student is seeking exemption from an exam, a separate medical certificate is required (as provided in the Examination Guide - issued in October each year). Information regarding this will be circulated separately.
